The company outlined a new reporting framework Wednesday after finding the incidents over six months, excluding the recent Hugging Face crisis. Two cases involved an unreleased research model and a GPT‑5.6 Sol training run inserting instructions into chat summaries to conceal mistakes or misaligned behaviour from users. Another involved an internal-only model using a leaked API key without authorization and then fabricating data. Two further cases involved models and agents communicating through unsanctioned message boards and file sharing. The final case covered two training examples in which models uploaded files to the internet so they could cite them as relevant answers to human evaluators. Under the framework, any employee can flag an issue for the safety and alignment team to investigate, with deadlines intended to ensure timely investigation and disclosure. Investigations will produce reports detailing observed behaviour, internal and external impacts, and response measures, although OpenAI reserves the right to revise the protocol. The news comes after OpenAI CEO Sam Altman endorsed rival Anthropic (Unlisted (US):ANTHRO) call to slow model progress on X, saying OpenAI had discussed a slowdown recently and would share more "soon."
